Please contact your Insurance Plan Coordinator to determine what portion of the fee is covered by your plan for mental health prior to your first appointment. This will include any co-pays or deductibles that are due at the time of service.

Not sure what to ask your insurance regarding mental health therapy benefits? The following information should be gathered:

•If your insurance plan covers therapy sessions with me.
•If you have a co-payment or co-insurance, and what that amount/percentage is.
•If your insurance plan has a deductible that includes mental health appointments. (A deductible is the amount you will pay out of pocket before insurance pays all or part of your appointment costs)
•If your plan will cover telehealth sessions.

Payment for Services

I accept the following for co-pays, deductibles, and private pay: Most major credit cards, HSA, Cash. Fees are due at the time of service.

Private Pay Rates

If you are seeking services and not using insurance the rates charged will be as follows:

Intake Assessment — 90791Billing code used for 1st session and then annually	$200.00
Individual Therapy — 90837 (53+ minutes)Billing code most often used	$180.00
Individual Therapy — 90834 (38–52 minutes)	$120.00
Individual Therapy — 90832 (16–37 minutes)	$90.00
Family without patient — 90846 (53+ minutes)Used upon client request	$180.00
Family with patient — 90847 (53+ minutes)Used upon client request	$180.00

Cancellations or Missed Appointments

Unless canceled at least 24 hours in advance, you will be charged $50.00 to the credit card on file at the time of your missed appointment. Please note that Moving Balance Therapy Services will provide clients who do not use insurance with a superbill in the event they choose to submit for out of network benefits.

Litigation Matters

I do not participate voluntarily in client litigation matters. If I am subpoenaed to appear as a witness involving a client, I will contact you to discuss the process. Charges associated with preparation, travel, and time I spend in court will be charged at a rate of $325.00 per hour including travel time to and from location. In addition, I do not make custody recommendations.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate (GFE) of expected costs before beginning services. You will receive a written GFE before your first appointment. For more information, visit our GFE page.
